- Visual Arts will not be held liable for any consequential action(s) resulting from or involving the use of borrowed department equipment. The individual bound in agreement with the terms and conditions set forth in this document is the sole trustee and guarantor of action(s) conducted with all Visual Arts equipment.
- You must be a registered Visual Arts student, faculty member or staff member.
- You must show a valid UWO student/faculty/staff card on request. Under no circumstances will equipment be loaned out should you be unable to present your card.
- All equipment will be signed-out for a period of 24 hours maximum with the exception of weekends defined as Friday afternoon through Monday morning. All items are due back the next day by 1pm. Exceptions will be made for holidays.
- Failure to return the equipment on time and in good working condition will result in the loss of future sign-out privileges as well as a $10/ day fine.
- Only in extreme circumstances (i.e. illness, etc.) will special exception be given to the person who fails to bring equipment back on time. You must contact the Operations Manager @ 519-661-2111 ext. 86190 in order to give notice that the equipment will be late.
- You are financially responsible for any damages the equipment may incur while signed out under your name unless it can be determined that the equipment was defective prior to being signed out. Please ensure that the equipment is functioning properly when signing it out and report any problems with the item.
Equipment Sign-Out Policy
The lending of equipment and other resources must be done judiciously. Accordingly, the first priority for all sign-out equipment is to accommodate course project and teaching demands. Extra curricular projects must be pursued as second priority; no guarantees can be made for the accommodation of these types of projects.
This reservation and sign-out policy is directed towards all Arts and Humanities sign-out equipment.
Students/Faculty/Staff may reserve equipment in advance by making a booking directly through the Sign-out booking website or in-person in VAC 244
Equipment dedicated for specific class use may not be signed out for general use through the term of the class.
Priority will be given to students enrolled in classes in which course projects require specific equipment.
Any student/faculty/staff who is not registered in Visual Arts or Film Studies cannot sign out any of the Arts and Humanities equipment.

The equipment can be picked up after 1pm on the day reserved and must be returned before 1pm of the designated return date (class scheduling exceptions should be made in advance).
All information provided by the borrower on the sign-out form will remain confidential unless s/he has violated any of the regulations pertaining to borrowing privileges.
Please Note: Students failing to respect the responsibilities listed, fail to return equipment in good order or fail to return equipment will be invoiced by the University for the full value of repairs or equipment. Be advised, uncollected amounts will be forwarded to a collection agency. Items not returned after one week will be considered theft and reported to the UWO police.
